Awake Mentoring Service: how to refer disabled people
You should:
- Discuss the service with the person you would like to refer. Ask permission from them to contact us on their behalf.
- Contact us on 01924 256999 and speak to a member of staff.
- Gain prior permission to provide us with the person’s name and contact details.
Awake Mentoring Service will:
- Contact the person direct and set up an initial visit to discuss their needs and answer any questions they may have.
- Carefully match the skills and experiences of a trained mentor to suit their goals.
- Give them an estimate of how long they will be on the waiting list.
- Introduce the mentor and the person to be mentored to check that they would like to work together.
- Let you (the referrer) know when mentoring support starts and ends.
- Keep you (the referrer) informed, within confidentiality guidelines, about progress.
Confidential information will only be shared with appropriate agencies with the person’s permission and with the aim of assisting them, except where it is considered essential for the welfare and safeguarding of a vulnerable adult.

The service also welcomes self-referrals, so if the person you speak to would like to contact us themselves, please ask them to phone 01924 256999 or email awakementoring@scope.org.uk
