Scope Cwmbran Adult Service
At Scope Cwmbran Adult Service, we support disabled people with cerebral palsy and other physical and learning impairments in Greater Gwent.
The core service is to individuals can choose and develop their individual lifestyles to lead independent lives.
This encompasses their health, care and social well-being, using a broad range of activities to enhance their daily lives.
People who attend the service are supported by having:
- A Person Centred Plan (PCP), balancing activities to support health, care and social well-being
- A keyworker
- An individual activity timetable
- A yearly service review, to assess people's needs are being met
