We are looking for a full time Community Support Worker to work in Intensive Support Part of the Dementia and Older People’s Mental Health Team based in Worthing West Sussex.

Intensive Support operates a to 9 to 5pm 7 days a week including Bank Holidays.

About the role

  • Assisting the community nurses to plan, implement, review and evaluate individual patients care plans, ensuring the patients’ needs are at the fore at all times. 
  • Carry out delegated duties as identified and designed by senior staff that related to the needs, changes and demands of the team and service user care, inclusive of the provision of personal and therapeutic care when required. 
  • Support the therapeutic relationship with service users through engagement and activities as deemed appropriate to the client group. Provide activity to groups or individuals to meet their recovery needs as identified by lead professional.
  • Record observations and changes to a service user's clinical conditions. 
  • Keep accurate, timely electronic and written records according to Trust policy, and data protection (GDPR) legislation. 
  • Empower service users to regain their health and independence in collaboration with other members of the multidisciplinary team. 
  • Undertaking clinical visits to patients and carers in the community, including as lone worker

Requirements

  • Good general education
  • NVQ level 3 or equivalent experience
  • Experience of working with mental health needs in a paid or voluntary capacity
  • Experience of working collaboratively with colleagues, wider teams, service users and so on
  • An understanding of confidentiality
  • Good communication skills with a good command of written and spoken English language
  • Ability to exchange factual information with service users using, reassurance, tact, empathy
  • Ability to work as part of a multi-disciplinary team, be flexible around shift work and have good communication skills
  • Ability to know when to escalate concerns
