Key information

Right now, we are looking for a Community Links Advisor to play a pivotal role in our Mental Health Service in Hammersmith and Fulham.

Sounds great, what will I be doing? As a Community Links Advisor, you will provide holistic, recovery-focused support to service users, helping them build independence, maintain their wellbeing, and sustain successful lives within their communities. 

About the role

  • Provide holistic, person-centred support to an allocated group of service users using recovery-focused tools such as SMART goals and the Recovery Star, with the aim of promoting independence, maintaining accommodation, and enhancing wellbeing.
  • Conduct regular one-to-one support sessions in service users' homes or community locations, including those who are hard to engage. 
  • Develop, implement, and review individualised support and risk management plans in collaboration with service users and relevant professionals. 
  • Support access to healthcare, community resources, and mainstream services, and encourage ongoing engagement with social and faith-based networks. 
  • Facilitate discharges from the Community Mental Health Team (CMHT), helping individuals build community connections and resources.

Requirements

  • NVQ Level 4 in Care or equivalent, or a minimum of two years’ experience in a mental health setting. 
  • Desirable: Lived experience of mental illness and completion of Peer Support training (or equivalent). 
  • Broad life experience with a positive, enabling outlook; understanding of the challenges faced by mental health service users. 
  • Knowledge and practical understanding of recovery, co-production, care planning, risk assessment, and recovery tools.
  • Strong written and verbal communication skills; confident in group facilitation and presentations. 
  • Able to assess situations quickly, work independently or collaboratively, and manage stress under pressure. 
  • Experience in coaching, mentoring, or delivering training.
