About the role

We are innovative in the way we deliver services to people and adopt the latest techniques in health and social care. This approach helps us to promote safe and high-quality services to meet the needs of people, their families and their carers.

Training, education and support for our staff is a crucial part of care innovation. We empower our staff to create their own ideas to make our services more responsive for the people who use them.

Some of our services include:

Home First

Our teams work with individuals to deliver shared person-centred goals, improving their quality of life and independence.

Stroke services

Our Stroke service works with people as they recover from stroke and receive rehabilitation in the community.

NHS@Home

NHS@Home is a new model of care that brings together health and social care providers from Acute Trusts and Community partners to deliver joined up care from the comfort of people’s own homes.

Learning Disability

Our Adult Learning Disability Health Service work with people to enable them to make choices about their health.

Public Health Nursing

The Public Health Nursing team works alongside all families with children aged 0-19 years. Our teams work with children, families and communities in their own homes and in schools to prevent ill health, protect health and promote wellbeing.

Children Therapy service

The Children’s Therapy service sits within a Multi-disciplinary team and we identify, assess, support and manage the health needs of children and young people aged 0-18 years.

Digital

We cover a wide portfolio from service desk and desktop support teams to clinical systems management and digital training.

Administration and Corporate

We’re looking for Administrators and Corporate Services staff to support our community services.
